Pricing is based on recipient’s information. There is no annual fee. Cost is one-time payment.

$100  regular membership cost -or- $10 if your recipient is self-identified limited, low or fixed income.

$50 for residents of 45402 45405 45406 45416 45417 45426 because Premier Health chips in the other $50.

You will receive an emailed receipt and a postal mailed greeting card and the member agreement your recipient needs to complete and return to activate their membership!
